Job : Programs are delivered in a holistic and culturally appropriate manner in various facilities and community locations. Coordinator works collaboratively with inter-professional teams across spectrum of care and is accountable for planning, quality monitoring and reporting. Coordinating, marketing, recruiting participants, facilitators, evaluating, collecting data and tracking of outcome measures is integral to program success.

- 2 years of community experience
Education
- Bachelor of Science in Nursing (BScN)
Other Education and Training
- Bachelor of Social Work is acceptable
- Bachelor of Psychology is acceptable
- Bachelor of Physical Therapy is acceptable
- Bachelor of Occupational Therapy is acceptable
- Bachelor in a related field is acceptable
