Second Mile Club of Toronto and PEACH

Established in 1937, The Second Mile Club of Toronto (SMC) is a program of the Kensington Health Centre that has provided community support services in Toronto for nearly 80 years.

We offer safe, recreational spaces with accessible and welcoming services for older adults and adults living with physical disabilities. Across multiple locations, The Second Mile Club provides drop-in Seniors Active Living Centres, Adult Day Programs, Case Management for clients living at home, Home Help, Transportation, and Caregiver Support.

The SMC is dedicated to enhancing the social, intellectual, emotional, and physical well-being of a multicultural community of older adults and adults living with physical disabilities. We strive to deliver safe and timely care with excellent health outcomes while fostering a respectful and inclusive environment where clients’ and members’ needs come first.

The PEACH Program is a collaboration among Inner City Health Associates, Kensington Health, and Toronto Central Home & Community Care Support Services. PEACH operates as an interdisciplinary, mobile, street- and shelter-based palliative care program serving people experiencing homelessness in Toronto. Through an anti-oppressive framework, harm reduction, and a trauma-informed approach, the PEACH Program aims to deliver equity-informed palliative care as a human right.

Position Overview

The Second Mile Club of Toronto (SMC) is seeking a  Health Navigator to provide case management and psychosocial support, assess client needs, and determine eligibility for services. This individual will conduct assessments as required and develop care plans based on clients’ needs and preferences on an ongoing basis.

Salary range:

$71,000 to $85,000 CAD per annum

Duration of contract:

Until May 27, 2027

Responsibilities:

• Lead strong community awareness and outreach through community partnerships, networking, and social media.
• Manage a clinical caseload while working closely with the PEACH Nursing Coordinator.
• Provide RAI-CHA assessments and direct social work services in accordance with social work clinical practice protocols and standards to clients and their families, and develop and facilitate appropriate and timely care plans and interventions.
• Liaise with families, clients, and other community agencies to enhance resources and communication for staff and clients.
• Provide strong advocacy, resources, and counselling to clients within the community.
• Represent the agency at outreach functions and work with multidisciplinary teams.
• Provide professional support and education to other team members in their work with clients and their families.
• Develop and maintain accurate documentation as per The Second Mile Club, PEACH, and Community Services standards and relevant regulatory bodies’ standards.
• Ensure professional development through reading current literature and attending relevant workshops.
• Demonstrate motivation for ongoing professional development through continued education.
• Educate clients and community partners on palliative approaches to care and harm reduction strategies.
• Attend health appointments with clients.
• Advocate for appropriate assessments, admissions, specialist consultations, OT/Physio, and addiction support for clients, both in hospital and in the community

Required Skills and Abilities:

• Strong organizational and coordination skills to facilitate a multidisciplinary approach to case management.
• Administrative and computer skills to complete professional reports, documentation, and statistical analysis.
• Excellent written and oral communication skills, along with strong organizational and interpersonal abilities.
• Ability to communicate effectively with clients of various ages, socioeconomic statuses, and ethnic backgrounds.
• Understanding of trauma-informed care, including knowledge of intergenerational trauma and its effects.

Required Knowledge and Experience:

• Minimum of three (3) years of social work experience with proven ability in community practice within the GTA.
• Experience working with structurally vulnerable or high-risk populations, including those experiencing homelessness, addictions, mental health challenges, palliative care needs, or living as seniors and adults with disabilities.
• Knowledge of and experience in social work practice in a community care environment, preferably with expertise in working with older adults and their families, and an understanding of the associated challenges of aging.
• Knowledge of community and government-based programs and services.
• Training and experience performing RAI-CHA assessments; knowledge of CIMS is an asset.
• Understanding of community initiatives such as CNAP, DWTC, interRAI CHA, and Resource Matching and Referral is an asset.

Required Professional Designation/Certificate:

• Master of Social Work degree from an accredited school of social work or equivalent, and eligibility for membership with professional associations (Ontario College of Social Workers and Social Service Workers).
• Strong skills in crisis intervention, palliative care, and mental health, along with an understanding of dementia and Alzheimer’s disease.

Other Requirements:

• Must have a very good attendance record.

Compensation:

Based on experience, plus a competitive benefits package.
